On arrival for your appointment you will be provided with a registration form. Please complete this in as much detail as possible prior to your appointment to help us get the most out of the consultation for you.
Payments will be processed on the day of your appointment. For details on our fee structure and concessions please see below.
If you need to cancel your appointment, you can do so through our website or by calling (03) 8554 0111 (bookings made through Platinum Physio reception).
Note: cancellations within 24hrs of appointment time may incur a cancellation fee
